An update for everyone - there have been some issues with Amazon PC customers not receiving the pre-order bonus DLC. Amazon actually chose not to participate in the pre-order bonus but our distributors Mastertronic have some codes left, so if you would like one then please contact support@mastertronic.com

Please note, that this is ONLY available if you're able to show proof of purchase.

I sent an email with proof of purchase to Mastertronic and got the code by email within 10 mins.

I also sent them an email, hopefully I'll get both codes since I ordered two copies.
Edit: Damn, they're fast! Received both codes exactly 11 Minutes after I wrote them.

Glad to hear that responsibility was finally taken.

Got a reply today, and have just winged The Fuhrer himself and then later finished him through the thigh (I was aiming for his funbag...!) So Thank You to Rebellion, and especially to Mastertronic for going beyond the call and providing us disappointed Amazon customers with codes regardless of their failings, I for one am very appreciative of your efforts and am a happy customer.
